EVALUATION

EVALUATION


"Performance"

There are 6 options for performance evaluations. The level of the evaluation required depends on the situation and your goals. 


The most basic level is a "Remote History and Testing". This include review of the client's history, review of systems and any questionnaires along with remote cognitive testing and "EQ" testing.


"Follow-Up" is for returning clients and for brief screening evaluation.


The "Performance Diagnostics" includes only diagnostic testing. NO physical testing is included in this option. It is a good start point for any new client. If warranted, a "Follow-Up" visit can be done on another day to add in physical evaluations. Some people prefer to break-up the comprehensive evaluation for fear that it is all too much for them at one time.


The "Comprehensive Performance Evaluation" is the option you need if want both diagnostics and physical evaluations.


The "Extensive Medical Evaluation" includes all of what is found in the comprehensive neurological exam and adds in a screen of autonomic neurological function (ANS). ANS measures provide a biomarker for both PTSD and PCS.


An "Extensive Medical Evaluation" + ANS stress testing and Holter monitor takes a deeper dive into the overall function of the autonomic nervous system, sleep, a client's capacity to recover, as well as the potential identification of environmental stress factors.


The "Neurological Capacity Exam" is an all day evaluation under physical and cognitive stressors, with a variety of outcome measures to assess the potential system fatigue and its' relationship to ADL.




PERFORMANCE EVALUATIONS:


There are also 2 options for the performance evaluations. These options are NOT for those who are suffering from an injury, or experiencing symptoms with the expectation to be treated. This is NOT a medical visit. Nothing is to be identified, diagnosed or treated. These evaluations are for overall fitness and health ONLY. The two evaluations cover BRAIN and BODY performance. The brain performance evaluation can also serve as a very comprehensive concussion baseline and should be considered for any athlete as well as those with a past history of concussions wishing to keep tabs on their level of function. NO MEDICAL SOAP NOTES for these evaluations will be created or provided.
